Registration Form Instruction

Your license must be current in order to fill out this registration form. If you attempt to registered with an expired license an automatic reply will inform you of the expired condition. If you are not sure, give the Pesticide Regulation Staff a call and they will assist you.

• Required: A current email address
• Required: A eight(8) character password with one special character, for example one of these; '! # ^ *'
• Required: Your ID that is on your green license
• Required:The Control Number that is on your green license
• The last 4 digits of your SSN
• Required: Your First and Last name that is on your current license
• Required: Your Phone Number where you can be contacted.
• Registration Comment(Optional Entry) to assist with your registration.

If your license is current continue filling out the form. Remember you password and then press the 'Register' button. If the system proves your information is good you will receive an email notification in your inbox.

Review your email inbox and follow the instructions from the received email.

Make sure you use an e-mail address that will have a long life for future communications from the Pesticides Staff. If you change your email address in the future please remember to contact that office staff so they can keep your record up to date.

You should find your License ID on your Applicator License. For example:Person ID: 123456

Enter the control number that is on the your license. The control number is located on the back of the license card at the bottom titled as ODA Control #.

Enter the last 4 digits of your SSN. Do not enter the entire SSN.
